An Invitation from: The Geriatric Planning Group

 Who: 

You are invited to join the Geriatric
Planning Group representing older adults,
community-based organizations, healthcare
providers, policy makers, public agencies and best
practice experts to understand issues and look for
creative solutions to address mental health issues
for older adults in the Commonwealth of Virginia.
What: During a one day summit you and other
participants will :

1. Learn how the geriatric planning group got
started and the goals of the summit

2. Learn from each regional group as well as state
agencies on what they are doing and sharing
best practices

3. Hear about a frame work of a continuous care
models for mental health geriatrics

4. Discuss the development of a plan to move a
cooperative model for geriatrics forward
Why: Your input will be the foundation for the
blueprint to build capacity and developing
initiatives and policies for a continuum of care for
geriatric mental health .
